In this episode, I welcome back Melbourne podiatrist James Ferrie, owner of My Sports Podiatrist. James first appeared on the podcast in Episode 375, where we discussed shockwave therapy and some clinical insights that many podiatrists overlook.
This time, we dive into an area that is rapidly gaining attention in musculoskeletal medicine: regenerative medicine and ultrasound-guided nerve hydrodissection.
For many podiatrists, cortisone injections and conservative management have long been the go-to approach for chronic pain conditions. But regenerative medicine offers a very different way of thinking about treatment.
Instead of simply managing symptoms, the goal is to stimulate the body’s natural healing processes and address the underlying degenerative pathology.
James explains that his interest in this field began more than 10 years ago, when he attended a prolotherapy course in Adelaide. What started as curiosity quickly turned into a long-term professional journey that has taken him around Australia and overseas to learn from leaders in regenerative medicine.
Understanding Regenerative Medicine
Regenerative medicine focuses on restarting the healing process in tissues that have become degenerative.
Conditions such as tendinopathy, plantar fasciopathy and osteoarthritis often involve degenerative changes rather than inflammation. Traditional treatments like cortisone injections can temporarily reduce pain, but they do not necessarily repair damaged tissue.
Techniques discussed in this episode include:
-
Prolotherapy
-
Platelet Rich Plasma (PRP)
-
Perineural Injection Therapy
-
Ultrasound-guided nerve hydrodissection
Each of these approaches aims to stimulate tissue repair or relieve nerve-related pain in a more targeted way.
The Role of Ultrasound in Modern Podiatry
One of the biggest shifts James describes is the increasing role of diagnostic and procedural ultrasound.
Ultrasound allows clinicians to:
-
Visualise soft tissue structures in real time
-
Identify nerve entrapments or degenerative changes
-
Guide injections with precision
-
Treat multiple structures within a complex pain presentation
James explains that ultrasound isn’t just about accurately placing an injection. It fundamentally changes how you assess musculoskeletal conditions.
Rather than focusing on a single pain point, clinicians can evaluate an entire anatomical segment and treat the structures contributing to the patient’s symptoms.
Nerve Hydrodissection Explained
One of the most fascinating topics in this episode is ultrasound-guided nerve hydrodissection.
This technique involves using fluid, often a 5% dextrose solution, to gently separate a nerve from surrounding tissue where it may be compressed or irritated.
Entrapment or constriction around nerves can lead to metabolic disturbances that trigger pain. Hydrodissection aims to release that compression and restore normal nerve function.
James explains that patients often experience significant relief shortly after the procedure, particularly in cases involving neuropathic pain.
Continuous Learning in Clinical Practice
A key message from this episode is the importance of continuous professional development.
James has spent years learning from doctors, radiologists and specialists outside the podiatry profession. By stepping outside the traditional podiatry conference environment, he has gained insights and skills that have transformed his clinical practice.
It’s a powerful reminder that the most interesting developments in healthcare often happen across professional boundaries.
Changing the Direction of Your Practice
Towards the end of the episode, we also discuss something many podiatrists struggle with: feeling stuck in the practice they’ve built.
If your diary is full of work that doesn’t challenge or excite you anymore, the reality is that you can change it.
Sometimes it starts with learning a new skill, introducing a new service, or simply blocking time in your diary for different types of patients.
Your clinic doesn’t have to stay the way it is today.
Final Thoughts
This episode is a fascinating look at how podiatrists can expand their clinical skill set and rethink the treatment of chronic musculoskeletal conditions.
Whether you’re interested in regenerative medicine or simply looking for inspiration to evolve your practice, there are valuable insights throughout this conversation.
