I’ve been to medical centres where they have sent me online forms to complete, only to have similar forms handed to me when I arrive. Or I will be handed a clipboard and asked to update my information, even though I can easily tell them there have been no changes in the past 12 months. 

The easier you make things for patients, the more positive their experience becomes.
